Typical review workflow?

We're working on refining our process for reviews and feedback and how those are communicated to animators and artists, and I wanted to get anyone's thoughts on how this might be improved, or how other people do this:

On a regular basis our director sits down with the team and a producer; they will work through a list of the "ready for review" versions on shots.

Usually all of the feedback is entered into a single note. All of this feedback is usually handled by a single artist or animator, so it makes sense to just enter it all together. However I know this means we can't treat each item as an individual thing - if the notes include, for example, five action items, they're all in a single note so can't be tracked separately.

My question is, what's the typical workflow for folks for this? Do people just assign notes to resources? Is it expected that individual tasks would be created for each item?

Most importantly we're trying to figure out a way to see notes from prior versions when reviewing subsequent ones so we know what was addressed without having to skip back and forth between versions. We also would love a system to track a status on each of the action items so we can tell which ones have actually been addressed and which ones have not.

Any insight would be most appreciated.
